In: Acute renal failure forum, edited by W. Lieberthal and S. K. Nigam. Am. J. Physiol. 275 (Renal Physiol. 44): F623–F632, 1998.—For more than 15 years, there has been an ongoing debate regarding the nephron segment(s) most severely injured in acute renal failure (ARF) induced by an ischemic or toxic insult. Although some investigators have argued that the proximal tubule (and particularly the S3 segment) is the major target of injury in ARF, others have held the view that damage to the distal nephron [particularly the medullary thick ascending limb (MTAL) segment] plays a more important role in this disease. In this discussion, the first of three on different aspects of ARF that have been hotly debated, we have invited several experts to discuss their opinions on this issue. The goals of this first discussion (and the subsequent two articles in this forum) are to establish areas of consensus in each area of controversy and also to identify unanswered questions that represent important areas for future research.

متن کاملImportance of RIFLE (Risk, Injury, Failure, Loss, and End-Stage Renal Failure) and AKIN (Acute Kidney Injury Network) in Hemodialysis Initiation and Intensive Care Unit Mortality
Our study evaluated the differences between early and late hemodialysis (HD) initiation in the intensive care unit (ICU) according to the RIFLE (Risk, Injury, Failure, Loss, and End-stage renal failure) and AKIN (Acute Kidney Injury Network) classifications. On the assumption that early initiation of HD in critical patients according to the RIFLE and AKIN criteria decreases mortality, we retros...
متن کاملRenal osteopontin protein and mRNA upregulation during acute nephrotoxicity in the rat.
BACKGROUND The effect of segment-specific proximal tubular injury on spatio-temporal osteopontin (OPN) distribution was determined in two different nephrotoxic rat models to evaluate its conceivability with a possible role for OPN in acute renal failure (ARF). OPN gene expression was further determined in proximal and distal tubular cells to investigate the origin of increased renal OPN. METH...
متن کاملEffect of aminophylline on renal function of brain injury patients with acute renal failure admitted to the intensive care unit
Introdution: Acute renal failure is a common complication in the brain injury patients. Released adenosine in injury can reduce renal glomerular filtration. Aminophylline is an inhibitor of releasing adenosine. So, this study aimed to evaluate the effect of intravenous aminophylline in renal function of brain-injured patients with acute renal failure admitted to the intensive care unit. Method...
